That's pretty much it. This is what I typed last year in order to pick up my sister's badge, and it worked:

I, [sister's name], do hereby grant my brother, [yours truly], permission to
pick up my Dragon*Con badge for me.

And just have your friend sign and date the permission slip. The yellow card definitely helps. And don't forget to get a photocopy of your friend's ID.
